- Persons 21 years of age or older diagnosed with Cystic Fibrosis
- Exclusive to Cystic Fibrosis Adults
- Patient must be affiliated with an Adult CF Center in Ohio, Kentucky or Indiana
- Patient must meet the medical criteria for a lung transplant as determined by their physician and transplant team
- Patient must be an approved candidate for a lung transplant
- Patient must be on a lung transplant waiting list
- Patient and/or patient's family must raise funds for "Love Your Lungs" foundation
- "Love Your Lungs" will match the patient's fundraising amount up to $1,000.00
- Maximum amount of financial assistance is $1,000.00 within a 12 month period
